Meeting For The First Time
It is extremely important that you fill out all of the new patient forms before we meet so that our first meeting can be extremely productive and informative. You can fill out the forms located using the "Patient Forms" link above or in the Important Links box below ahead of time or in person at our offices. If you have any questions about the forms please come in and see us or you can contact us by phone, e-mail, or online form.
It is important for us that our new patient health practitioner relationship is a successful partnership built on mutual trust, security, respect, and understanding. The goal of this new relationship is your successful treatment and oral health care. Part of this is understanding what you as the patient has set for oral health goals. We will talk about these goals at length in person during your first visit and assessment. Another important part of this new relationship is gaining knowledge about you, your current health and your past health. Because of this it is very important that you bring with you all relevant dental health records on yourself. This includes files, images, x-rays (not photo copies of x-rays, photo copied x-rays are fairly useless), and other related information. We request that you bring this information with you in paper form for our records. If this is your first time going to the dentist don't worry about not having these records because over time we can help build this information.
The American Dental Association recommends regular screenings, cleanings, assessments, x-rays, and other diagnostic tests. Some of these are recommended yearly (teeth cleaning) while others are recommended every couple of years (full dental x-rays). During your first visit we will figure out what is necessary for you to have performed. During this first visit we will also diagnose any oral health issues that you may be having, the conditions that may be present in your mouth, as well as formulate a concise and detailed plan for your oral health treatment.

After our first meeting, initial examination, and assessment we will work together based on your oral health and goals to develop a plan for your oral health treatment both in the near future and long-term.
